1) Have a pre-trip check. Make sure your blood sugar levels of A1C, blood pressure and cholesterol levels are fine. Get vaccinations for any country you plan to visit.
2) Wear a medical identification of diabetes. Ideally it should be in the language spoken in the country you are visiting. Not everyone speaks your language and not to medical problems through misunderstandings.
3) Keep your blood sugar in medication and snacks in your carry-on luggage. Check-in baggage, unfortunately, are going the wrong way. Do not risk your diabetes medication in your main luggage packing.
4) Keep your medications in their original box, labeled by the pharmacist. To avoid misunderstandings about why you are taking drugs and, if you are on insulin, syringes.
